Dear Mr. Becker:
I am writing to submit my resignation from Capital Hospice, effective November 14, 2009. Although the past five years have been very rewarding, I have been offered a position at American Hospice as Director of Social Work and feel it is time to move on and accept this new challenge.
Before I leave, I would be happy to develop a transitional plan so my replacement has the information he or she needs to serve in this role. Please let me know if you would like me to draft this plan.
Thank you again for the opportunities you have provided. I appreciate having had the opportunity to work at Capital Hospice and manage such a hardworking and dedicated team of social workers.
